Parent Permission Form to Take Student Home

In order for students to go home with their parent/guardian after a co- or extra-cuuricular event, the form below must be:

  1. Signed by the parent/guardian
  2. Signed by the advisor
  3. Signed by Assistant Principal Joe Cusenza, preferably with 24 hours notice
