Parent Permission Form to Take Athlete Home

In order for student-athletes to go home with their parent/guardian after an athletic contest, the form below must be:

  1. Signed by the parent/guardian
  2. Signed by the coach
  3. Signed by Assistant Principal Joe Cusenza, preferably with 24 hours notice
